How to Prepare for your Appointment

The duration of your appointment can range from 45 minutes to 1.5 hours.

Please arrive 5-10 minutes before your appointment time to allow sufficient time to check-in and register.

WHAT DO I NEED TO BRING?

Any eye wear that you are currently wearing - this includes any driving, computer and reading glasses, sunglasses, full time wear glasses, safety glasses or sports eyewear
If you wear contact lenses please bring the box or a blister pack with your current brand and lens information
A list of your current medication
A valid Ontario Health card
If you have additional insurance, please bring your card or information with you (Please check your eligibility prior to your arrival as we are unable to do )
If you require completion of any forms or third party requests e.g. Ministry of Transport licence, employment or volunteer forms, please advise us ahead of your appointment

Late Cancellation and No Show Policy

Thank you for choosing our clinic. We take the time to prepare for your visit. If you are unable to attend your scheduled appointment, please notify the clinic at least 24 hours in advance otherwise you will be subject to a $75 fee.
